Procedures Document (updated 12/2012)
If your institution would like to do business with ODE, you must apply for an institution identification number. Please save the document to your own system, fill out the form, and email it to the ODE Institutions Database Maintenance Team Joy Blackwell We will contact you about your request. You may also use this form to update your existing institution’s information or to close your institution. Institution requests (new/changes) must be accompanied by school board minutes authorizing the new institution or change. |

Oregon School Directory
The Oregon School Directory is an online directory listing phone numbers, addresses, and key staff people in all public schools and districts in the state. It further contains information on Oregon Department of Education staff, additional information about private schools, and statistics on the number and composition of Oregon schools. |
