RE: Process for Annual Reporting of Sanitation and Fire Safety Inspection Dates
Action Required: Sanitation and Fire Inspection Date Certification form due by January 19, 2007
The Oregon Department of Education Child Nutrition Programs (ODE CNP) records show your program has at least one license-exempt site participating on the Child and Adult Care Food Program (CACFP). USDA and ODE CNP require sponsors with CACFP sites that are exempt from Child Care Division (CCD) licensing to receive a satisfactory sanitation inspection every year, and a satisfactory fire inspection every other year.
Enclosed is a list of sites that are license-exempt according to ODE CNP records. Complete and return the enclosed Sanitation and Fire Inspection Date Certification form to ODE CNP by January 19, 2007.
Next to each site name write in the dates the site received the last sanitation inspection and fire inspection. The sanitation inspection must have been conducted within 12 months from the date you submit the form. The fire inspection must have been conducted within 24 months from the date you submit the form.
If a site is inactive, do not submit the inspection dates. Please note the date each site became inactive in the column labeled
Inactive Site Effective Date.
Do not submit dates for sites not listed on this form.
Do not submit the fire or sanitation inspection documents.
If the inspections have lapsed and you will not have the required inspections before January 19, 2007, submit the completed form with copies of your sanitation and fire inspection request letters.
Sign, date and return the form to ODE CNP no later than January 19, 2007.
ODE CNP recommends sponsors immediately review the sanitation and fire inspection report dates for each license-exempt site to determine if new inspections are required before submitting the certification. Sanitarian and fire inspectors need to receive the inspection request as soon as possible.
