Effective July 1, 2012, Child and Adult Care Food Program (CACFP) sponsors are to use the following forms and information to determine CACFP income eligibility and develop the One Month Attendance Report (OMAR).

For additional languages, sponsors may use the USDA-developed materials available on the USDA website: http://www.fns.usda.gov/cnd/care/Benefit_Forms/Translations.htm

Important Notes:

Please recycle unused forms from the previous year and replace them with the current forms.
The CIS “Sponsor Use Only” section is on the first page. Copy both sides of the CIS for distribution.
Download the Letter to Household from the ODE website and insert the appropriate information in the areas of bold brackets, prior to distribution.

Income determinations based on the CIS will be valid for 12 months. For more information, refer to Chapter 21 in the USDA CACFP Center Manual “Big Red”.

If you have questions or do not have Internet access and need a translated form, please contact your assigned Child Nutrition Specialist.
