RE: Child Enrollment Form Policy - Questions and Answers

The enclosed document  answers questions regarding the Oregon Department of Education Child Nutrition Programs (ODE CNP) Child Enrollment Form Policy memo dated May 18, 2007. The memo detailed the policy that requires sponsors to have parents complete the child enrollment form to show their child is enrolled in the Child and Adult Care Food Program (CACFP).

ODE CNP implemented the Child Enrollment Form policy per the Federal regulation and USDA guidance.
