TO: Child Care Centers, Adult Day Care, Head Start, At Risk, Homeless and Outside School Hours Programs participating in the Child and Adult Care Food Program

RE: Revised List of Recognized Medical Authorities

Effective immediately, the Oregon Department of Education Child Nutrition Programs (ODE CNP) has expanded the list of medical authorities.

ODE CNP recognizes the following medical authorities: Licensed Physicians (MD), Physician’s Assistants (PA), Registered Dietitians (RD), Nurse Practitioners (NP), Registered Nurses (RN), Naturopathic Physician (NP), Doctor of Osteopathy (DO), and Naturopathic Doctor of Osteopathy (NDO).

Enclosed is an updated Medical Statement for Food Substitutions  (Form 581-1087-E) located in the Child Care Center Manual (“Big Red”) Exhibit 23.24. Please replace this revised form in your manual.
