RE: Expiring Confidential Income Statements and CACFP Child Enrollment Forms
The Oregon Department of Education Child Nutrition Programs (ODE CNP) received the following guidance clarification from USDA concerning the expiration date for Confidential Income Statements and CACFP Child Enrollment Forms.
Confidential Income Statements:
Part 226.23(f) states: free, reduced price and paid meal eligibility figures must be reported by institutions to State agencies at least once each year and shall be based on current family-size and income information of enrolled participants. Such information shall be no more than 12 months old.
CACFP Child Enrollment Forms (not applicable for Adult Day Dare Sponsors):
USDA regulations require that CACFP Child Enrollment forms must be completed annually by the parent or legal guardian
The intention of these requirements is that Confidential Income Statements and CACFP Child Enrollment Forms are current and valid until the last day of the month in which the form was dated one year earlier. This means that if a form was dated September 12, 2009, it is considered valid until September 30, 2010.
