TO: Sponsors of the National School Lunch Program
RE: On-Site Reviews for School Year 2009 / 2010
The Oregon Department of Education Child Nutrition Programs (ODE CNP) annually
informs sponsors of the National School Lunch Program (NSLP) the On-Site Review
requirements.
On-Site Reviews evaluate lunch counting and claiming systems. Every NSLP sponsor,
with more than one dining site, must complete an
On-Site Monitoring Form 
for each
dining site on or before February 1, 2010.
In order to complete an On-Site Review, it is necessary to:
- Observe the lunch counting and claiming process for every dining site serving
school lunches
- Complete the enclosed On-Site Monitoring Form (Form 581-3731h-P) for each site
observed
- Retain completed form(s) at the local level for three years, plus the current school
year
