2009/2010 Confidential Income Statements, Letters, and Income Eligibility Guidelines

Effective July 1, 2009, Child and Adult Care Food Program (CACFP) Sponsors of Family Day Care Homes are to use the following forms and information:


NOTE: The Confidential Income Statement, Section 4 (Household Members & Monthly Income) was revised to include the column Check if No Income.
