Site Monitoring Report for CACFP Revisions

The Oregon Department of Education Child Nutrition Programs (ODE CNP) revised the Site Monitoring Report for Child and Adult Care Food Program (CACFP) center sponsors.
The Site Monitoring Report for CACFP revision includes information concerning Other Federal, State or Local approval, and the Building for the Future Poster.
Effective immediately, use the enclosed
Site Monitoring Report for CACFP 
when monitoring sites. Discard all other unused versions of the form. Please inform site monitors of the changes in the form and provide a copy of the revised form for their use.
