TO: Sponsors of Family Day Care Homes

RE: Child Enrollment Policy Modification effective May 1, 2007

The Oregon Department of Education Child Nutrition Programs (ODE-CNP) is modifying the child enrollment policy for Family Day Care Home Sponsors participating in the Child and Adult Care Food Program (CACFP) in order to be in full alignment with USDA guidance.

Effective May 1, 2007, Family Day Care Home sponsors must implement procedures that ensure compliance with the following child enrollment policy:

  1. Each child in care must be enrolled in the CACFP.
  2. A completed child enrollment form is required for every enrolled child, including the provider’s own. (NEW POLICY)
  3. Enrollment forms must be signed and dated by a parent or legal guardian.
  4. Enrollment forms must be updated annually.
  5. Routine child schedule changes do not require an updated child enrollment form (NEW POLICY)
  6. The effective date for child enrollment forms is the latter of the two dates: 1) first day of the month in which the form was signed by the parent or legal guardian; or 2) date noted on the enrollment form for the child to begin care. (NEW POLICY)
  7. The enrollment form must include information on each child’s normal days and hours of care, and meals normally received while in care.
  8. The child enrollment form is intended to be used only as a guide to determine the usual days and hours of care and meals normally received while in care. Meals will not be disallowed from a claim based solely on information on the enrollment form.
  9. Enrollment forms must be maintained on file by the sponsor and the provider. (NEW POLICY)

The child enrollment policy is intended to better reflect the actual process family day care home providers use when enrolling new children on the program, and increase the accuracy of the information by having parents indicate normal meals and hours in care instead of all meals and all possible hours to “cover” any possible schedule change.

ODE CNP is aware that many sponsors use the enrollment form to capture the parent or guardian’s address, home and work telephone numbers used for household contacts. Nothing in this policy is meant to prevent the updating of this information as it changes. This policy does not apply to provider claims processed prior to the receipt of this memo. Prior claims may not be adjusted as a result of this policy.
