Supplemental 2012-13 Report Card Data Collection - Final Update

Deadline: August 30, 2013 is the deadline to submit the completed 2012-13 School and District Report Card Supplemental Data Collection templates in both English and Spanish.

The completed templates, in both English and Spanish, must be submitted to ODE as files using the Secure File Transfer Process. The templates may be zipped and submitted as a district. Process for Missing Report Card Data: English and Spanish versions of the Superintendent and Principal letters, as well as the school curriculum and learning and district curriculum and environment information (Racial Equity in Hiring and Student Wellness Policy), must be received by August 30, 2013.

Executive Numbered Memo: 012-2012-13 - New Oregon Report Card, states:

The Superintendent and principal letters, as well as the school curriculum and learning and district curriculum and environment information, must be submitted in both English and Spanish. Districts are responsible for producing and providing the Spanish translation for this submission. If a school fails to provide any of the information above, a notation in the Report Card will appear indicating that data was not submitted. If a Spanish translation is not provided, both the English and Spanish sections will be notated as not being submitted, regardless of whether an English version was submitted.


As stated in the memo, if a Spanish translation is not provided, both the English and Spanish sections will be notated as not being submitted, regardless of whether an English version was submitted. The following notations will appear in these sections if both and English and Spanish versions are not submitted:

Letters from the Principal and Superintendent (All Report Cards)
“A letter was not submitted for this school/district.” (The word “school” should be used on school report cards, and the word “district” should be used on district report cards.)

Racial Equity in Hiring (District Report Card)
“This area was intentionally left blank as the district did not provide a statement or may not meet student population diversity to require the statement be provided.”

Student Wellness Policy (District Report Card)
“This area was intentionally left blank as the district did not provide a statement or may not meet the minimum criteria to require that a statement be provided.”

Curriculum and Learning Environment (All Report Cards)
“Information was not submitted for this section.”
