|Institution Update Deadline September 14, 2012|
Institution update changes for the 2012-13 school year must be submitted to ODE by September 14, 2012. Please allow up to four weeks processing time.
The institution update form is located at http://www.ode.state.or.us/search/results/?id=227. Scroll down to the Institution/School Directory Updates link in the center of the page near the bottom. Refer to Appendix C for required documentation.
Changes to staff, phone numbers or email addresses only, may be submitted on the Appendix A - School Directory Worksheet. All other changes must be made using the Institution Update form and corresponding paperwork.
